    Psychosocial Assessment & Therapeutic Intervention

At Create Allied Health Services, we provide social work for cases involving mental health barriers, trauma, adjustment challenges, or psychosocial disability. We understand the complexity many clients face navigating mental health, grief, relationship breakdown, or major life transitions—and we're here to provide therapeutic expertise.

Our services are trauma-informed, evidence-based, and focused on clinical outcomes. Whether you need comprehensive psychosocial assessment, crisis intervention, or recovery planning, we bring depth of expertise to support your clients.

    Hospital Discharge & Rehabilitation

Whether your client is recovering from serious injury, facing delayed discharge due to psychosocial barriers, navigating complex family dynamics, or requiring community reintegration support, our team provides rapid clinical response with expertise and clarity.

Create Allied Health Services provides iCare-funded and hospital-supported social work for urgent discharge planning, mental health stabilization, service coordination, and post-hospital community transition. Our person-centered approach addresses the clinical complexity preventing safe discharge—mental health, capacity concerns, housing barriers, family conflict.

We collaborate with hospital discharge teams, allied health managers, rehabilitation specialists, and medical staff to ensure transitions are clinically sound, timely, and sustainable.

    Housing & Accommodation Support

We provide expert housing navigation for cases involving crisis homelessness, NCAT applications, guardianship complexity, mental health barriers, aged care placement, or multiple failed housing attempts.

Our team works with iCare case managers, NDIS support coordinators, hospital discharge planners, and aged care providers when standard housing navigation isn't sufficient—when clinical assessment, professional advocacy, capacity evaluation, or therapeutic intervention is required.

Whether your client faces housing crisis, needs SDA/SIL transition support, requires professional references that carry clinical weight, or needs aged care placement coordination, we provide the expertise standard housing workers cannot offer.

    Guardianship & NCAT Support

We provide specialist expertise in NSW guardianship law, NCAT applications, capacity assessment, and tribunal support for accommodation, health, and lifestyle decisions.

Our team works closely with case managers, hospital discharge teams, support coordinators, aged care providers, and legal professionals to prepare comprehensive applications, gather clinical evidence, and coordinate with appointed guardians.

Whether your client requires urgent capacity assessment for hospital discharge, guardianship application for housing decisions, tribunal reports for medico-legal proceedings, or supported decision-making coordination, we deliver court-quality clinical expertise.

  • Aged Care Transitions

We provide specialist social work for complex aged care transitions—hospital-to-aged-care pathways, NDIS-to-aged-care navigation, young people in aged care requiring transition support, and residential placement decisions involving family conflict or capacity concerns.

Our Clinical Director is a PhD candidate researching social work roles in aged care settings and a member of the AASW Aged Care Working Group. This specialist knowledge informs our practice when your clients face the complexity of aged care decision-making, ACAT processes, provider matching, or adjustment to residential care.

  • Clinical Supervision & Professional Development

We provide AASW-registered clinical supervision for social workers, placement supervision for students, and case consultation for allied health professionals managing complex psychosocial presentations.

Our supervision is reflective, evidence-based, and grounded in real-world practice across mental health, guardianship, aged care, housing, and complex case coordination.
